LIFEPlan is a provider-led Care Coordination Organization/Health Home serving individuals with intellectual and developmental disabilities in 38 counties across Central and Northern New York, the Capital Region, Hudson Valley and Southern Tier. LIFEPlan CCO NY, LLC, was founded by 68 agencies who are recognized leaders in the provision of care to people with intellectual and developmental disabilities.

LIFEPlan CCO is looking for a talented and committed person to lead our Member and Family Advisory Council!

The Manager of the Member and Family Advisory Council will support the mission of LIFEPlan through outreach and relationship building with members and families. The Manager will lead the efforts of regional Member and Family Advisory Councils while understanding the needs of members and families, providing educational programming, and promoting activities that support the LIFEPlan membership throughout the full geographic scope of LIFEPlan CCO.

The Manager supervises Family Liaison staff.


Education/Work Experience:

  • Bachelor’s degree required – in Health and Human Services, Education, or related fields preferred, plus a minimum of 5 years relevant work experience required.
  • Qualifications in the areas of disability services, social work or counseling, management, education, marketing and communications would be advantageous, as would a proven track record of developing strong relationships with a diverse range of people. Demonstrated ability to lead groups and affect change.
