The Path to Comprehensive Health Home Care Management 

LIFEPlan is currently working with both OPWDD and the Department of Health to define the role and mission of an IDD Health Home. Care Coordination Organizations (CCOs) are specialized IDD Health Homes that work with people with intellectual and developmental disabilities and their families to coordinate health AND developmental disability services*.   

The Department of Health’s goals for NYS Health Homes, including CCOs, are focused on achieving the Institute for Healthcare Improvement “Triple Aim”, the simultaneous pursuit of improving

1. The patient experience of care
2. The health of populations
3. And reducing per capita cost of health care.

To ensure a complete and successful transition from Medicaid Service Coordination to Comprehensive Health Home Care Management LIFEPlan is actively investing in the development of an integrated clinical team, specialized training, and other dedicated resources to support our care managers and members. 

 *The New York State Health Home program was created to make sure its members with chronic conditions get the care and services they need. This may mean fewer trips to the emergency room or less time spent in the hospitals, getting regular care and services from doctors and providers, finding a safe place to live, and finding a way to get to medical appointments (adapted from the NYS Department of Health, Health Home, Website).
