LIFEPlan CCO is one of six care coordination organizations approved across the state with at least two serving every county. Starting July 1, these organizations began coordination of all services for people with developmental disabilities — including disability services, health care, mental health care and support services.
This holistic care management represents a new direction for the state. Previously, individual agencies provided Medicaid service coordination that only covered disability-related services and families had to navigate other kinds of services on their own.
