Transportation is based on individual needs. Families have been individually consulted to determine if they are able to transport with reimbursement from program or if this is not an option ACHIEVE is providing transportation utlizing agency vehicles. No buses are being used to help ensure safety.
Achieve's Day Hab program opened on 8/3/2020 with 35 members returning to program at the Broome County location (125 Cutler Pond Rd, Binghamton, NY 13905) and 11 members returning to Chenango County location (214 Country Club Lane, Norwich, NY). More members will be returning next week. All members that returned this week reside in the community or in Family Care Homes; next week members who reside in agencies outside of ACHIEVE may be returning. Those members that are unable to return to site-based are receiving services within their homes to meet their individual needs. Due to safeguards in place there is no lunch times at Day Hab as program is only four hours long at this time. Those members with high medical needs and increased risk of contracting COVID have not returned to Day Hab at this time.

At this time, families will be asked to transport individuals to and from the Day Program whenever feasible to assist in reducing risk. The Day Program will work with families who have a hardship in doing this. Individuals attending different day programs will not be transported together. When individuals who live in IRA's return to Day Program, the residential facility will be asked to transport whenever possible. The Day Program will work with Residential Providers who have a hardship in doing this. Arrivals will use the front door of the program and will be brought in individually by employees following their screening and hand hygiene. Departures will use the side door and will be brought to the vehicle individually. During transportation, all employees will wear face coverings and individuals will be supported to do so; windows will be rolled down, weather permitting; seating will be spaced and vehicle capacity will be at 50%. Individuals who cannot/will not tolerate the use of face masks will be transported alone whenever possible. If they must be transported with others, they will sit as far away as possible near an open window. The vehicle will be cleaned in between uses. Cleaning supplies will be stocked in the vehicle in a locked container.
At this time, we plan of offer an a.m. and p.m. session of DH services in order to be able to provide service to as many people as we can. Individuals will be in static groups and assigned to a specific classroom. Any activities scheduled in common areas will be done with only one group at a time and the common area will be cleaned in between uses. Activity supplies will not be shared as much as possible and sanitized frequently and in between use. Fire drills will be held quarterly and implemented in both the am and pm sessions. Social distancing and staggered departure from the program will not be possible during the evacuation as it is critical that individuals understand the need to leave immediately upon hearing the alarm. Social distancing will be employed at the meeting area as much as possible, while ensuring safety and supervision levels. Groups will not intermingle. Prior to entry to the facility, a health screening will be conducted for individuals, employees and visitors that must be passed. Anyone who fails the health screening will not be admitted to the facility and a written information sheet will be provided regarding health care and testing resources. In cases where program staff provide transportation, the health screening of individuals will be done upon pick up. Individuals who fail the screening will not be transported to program. The supervisor, team leader, R.N. or LPN will conduct a review of the daily health screenings in accordance with OPWDD guidance. Staff will be trained on reporting, symptoms, exposure, etc. A return to program note from a physician will be required for any individual who has tested positive or has symptoms consistent with Covid-19. If the individual is quarantined as a result of a member of their household testing positive, a release from quarantine from the DOH will be required prior to return to program. Employees who test positive will follow the return to work guidance prior to returning to the site. In the event that an employee, visitor or individual develop symptoms during the program day, the employee or visitor will be asked to leave immediately. Individuals will be brought to a separate room away from all other individuals, contact made to their care provider and monitored until transport home. Appropriate reporting to OPWDD and the LHD will be done in accordance to current guidance. The number of individuals assigned to each classroom will be based on the square footage of the room, taking into account furniture, room shape, etc. Markers will be placed on the floor to assist staff and individuals to maintain spacing. Where possible, specific bathrooms will be assigned to specific classrooms. Individuals will remain in their classroom during transportation time and brought individually to their transport to reduce traffic in the hallways. The front door will be used for transportation drop off and the side door used for transportation pick up. Common areas will be used by only one classroom grouping at a time and cleaned in between uses. Staff break areas will have capacity limitation and breaks will be staggered accordingly. Supports will be provided to individuals accordingly to practice social distancing. *In terms of intakes, it will depend on the person’s situation, as well as the benefits of attending a structured program and the risks involved. . We have opened our locations first to people living with their families, living in Family Care or living independently. We are still determining people returning, etc. After that we will be looking at people living in certified IRAs.

At this time, we plan of offer an a.m. and p.m. session of DH services in order to be able to provide service to as many people as we can. Individuals will be in static groups and assigned to a specific classroom. Any activities scheduled in common areas will be done with only one group at a time and the common area will be cleaned in between uses. Activity supplies will not be shared as much as possible and santizied frequently and in between use. Fire drills will be held quarterly and implemented in both the am and pm sessions. Social distancing and staggered departure from the program will not be possible during the evacuation as it is critical that individuals understand the need to leave immediately upon hearing the alarm. Social distancing will be employed at the meeting area as much as possible, while ensuring safety and supervision levels. Groups will not intermingle. Prior to entry to the facility, a health screening will be conducted for individuals, employees and visitors that must be passed. Anyone who fails the health screening will not be admitted to the facility and a written information sheet will be provided regarding health care and testing resources. In cases where program staff provide transportation, the health screening of individuals will be done upon pick up. Individuals who fail the screening will not be transported to program. The supervisor, team leader, R.N. or LPN will conduct a review of the daily health screenings in accordance with OPWDD guidance. Staff will be trained on reporting, symptoms, exposure, etc. A return to program note from a physician will be required for any individual who has tested positive or has symptoms consistent with Covid-19. If the individual is quarantined as a result of a member of their household testing positive, a release from quarantine from the DOH will be required prior to return to program. Employees who test positive will follow the return to work guidance prior to returning to the site. In the event that an employee, visitor or individual develop symptoms during the program day, the employee or visitor will be asked to leave immediately. Individuals will be brought to a separate room away from all other individuals, contact made to their care provider and monitored until transport home. Appropriate reporting to OPWDD and the LHD will be done in accordance to current guidance. The number of individuals assigned to each classroom will be based on the square footage of the room, taking into account furniture, room shape, etc. Markers will be placed on the floor to assist staff and individuals to maintain spacing. Where possible, specific bathrooms will be assigned to specific classrooms. Individuals will remain in their classroom during transportation time and brought individually to their transport to reduce traffic in the hallways. The front door will be used for transportation drop off and the side door used for transportation pick up. Common areas will be used by only one classroom grouping at a time and cleaned in between uses. Staff break areas will have capacity limitation and breaks will be staggered accordingly. When we re-start our Community Based Prevocational Services we will not be having participants go to any centralized locations--the majority of sites we will be using are out-door sites, such as State Parks, where we perform training an paid work, and at our agency flower and vegetable garden. We will only work at indoor community sites where social distance spacing can easily be accommodated and will provide signage to those locations if they do not already have it. But any in-door programming during the summer months will be considered on a very limited basis. Community prevoc has developed a transportation plan, ensuring that only individuals and staff from the same cohort will be transported together. When transporting individuals we will reduce capacity on vans to 50% of total capacity (3 individuals plus the staff-driver). Groups use of the hub site will be staggered with one using it in the AM and another in the PM, while the majority of service time will be in the community. Hub sites will be disinfected between groups. Community Prevoc is using mini vans for transport with a maximum occupancy of 3 individuals and staff-driver. The van will be seated from back to front and exited from front to back to minimize individual's proximity to each other. Only one individual will sit in a row. Staff will be required to wear masks when transporting individuals. Staff who cannot medically tolerate use of a face mask will not be assigned to assist with transportation. Individuals who can tolerate a mask will be coached/supported to wear their masks while riding. Vehicles will be cleaned and disinfected by each respective driver at the start of the shift, in between individuals/groups and at the end of day. Staff will roll down windows in vehicles as is safe and appropriate to permit air flow.
CWI will maintain visitor logs identifying the name and contact information for all visitors. A Community Prevoc managers have been identified as the health and safety site monitors for this service and will file all health screenings daily. CWI has policies and procedures in place for the reporting of any individual or staff who has received a positive COVID-19 test result. The respective manager notifies the CWI Quality Assurance staff, who in turn notify the local health department and OPWDD. In the event of a positive test result, Community Prevoc Managers and QA staff will be required to trace all contacts in the workplace through building sign-in logs, health assessment forms and KRONOS (electronic time keeping system). Managers will provide the CWI Quality Assurance staff with the list of contacts and QA staff will then notify the health department of all staff, individuals and visitors who entered the facility dating back to 48 hours before the positive person began experiencing COVID-19 symptoms or tested positive, maintaining confidentiality as required by federal and state law and regulations.

CBPV services are provided as a 1:1 service unless the individuals reside together and then 2 people may receive CBPV service at the same time with 1 staff person. All individuals must wear face masks when in a vehicle. Before providing services with an individual, staff are disinfecting high touch points (such as door knobs, gear shift, steering wheel, window buttons, radio buttons, etc). Then, after each individual's service is complete, support staff are disinfecting the vehicle again. Support staff have been encouraged to roll the windows down to permit air flow whenever possible.
Staff and individuals must wear face masks whenever social distancing cannot occur. There are COVID sanitizing kits and hand sanitizer in all of the vehicles (for their personal cars and agency vehicles). There are extra face masks available for individuals if they do not have one. Gloves are also available and support staff are aware of how to properly remove gloves. There are garbage bags in the vehicles for staff to discard used gloves. In June 2020, all support staff completed the Pandemic Prepardness Training offered by NY Alliance and their certificates of completion are in their personnel files. While providing community based PV services, all staff are aware of the importance of hand hygiene and/or use of hand sanitizer. Staff will ensure that individuals are washing their hands whenever possible. There are bottles of hand sanitizer in all of the vehicles and support staff are aware of where to obtain more hand sanitizer when needed.

A meeting was held with our transporation vendor on 7/21/20. Ryan Oakley, First Transit Supervisor will be overseeing and implemetation of all services provided during transportation. DH Director and Associate Director will be in close contact with First Transit weekly to follow up with any questions or concerns that may arise.
Currently our DH has staggered start and end times and that will continue. Our services are also based upon teams with the same staff and consumers on a daily basis. All clubs, therapies and group activities will be put on hold and common areas will no longer have seating available or closed off if possible (snoezelen room). Residences will be reminded that all food needs to come in prepared properly (fine ground, puree, etc). Staff have been reminded that all utensils and containers need to be put through the dishwasher and can not be hand washed. Staff will be reminded that during breaks they must maintain social distancing or masks must be worn and they will be told that there should not be more than decided upon number in the smoking area at any given time. All staff have received their sanitation kits and instructions on use at the meetings on July 8-10th. Extra face masks for both staff and consumers are available as are hand sanitizers and other PPE such as face shields and bodysuits. Some team rooms are equipped with sinks which will reduce roving throughout the building; for those that are not they will be directed to use the nearest sink/restroom. Hand sanitizer has also been distributed to each staff member and all vehicles are also stocked with sanitizer and cleaning products. Staff will be provided additional training on hygiene and cleaning prior to the return of consumers. Staff will also provide hand sanitizer to each consumer as they enter the building. Each team room will maintain their own cleaning logs which will indicate date, time and scope of cleaning completed. These will be handed into the Safety Monitors on a weekly basis. All windows will be open as applicable and staff will be reminded daily to make sure they are shut and locked at the end of the day. All items not needed for programming will be prohibited from entering into the West Rd building. Consumers will be asked to the extent possible to leave all personal belongings at the DH facility. If clothing is soiled it will be returned home for cleaning and additional clothing will be brought in. A letter will be sent to all residences explaining this process.

Staff's vehicles are currently limited to only 2 individuals being transported at a time, unless a van is used and then three individuals may be transported (50% capacity reduction unless they are coming from the same home). Individuals that are transported together will remain in the same group together. Health assessments will be completed prior to entering the vehicle. Individuals and staff will be spread out in the vehicle as much as possible and they will wear a mask at all times, unless not medically able to do so. Staff will always wear a mask in the vehicle, and will not be assigned transportation if they are not able to do so. If individuals cannot wear a mask, they will be transported alone or only with members of the same household. High-touch areas of the vehicles will be cleaned/disinfected at the beginning and end of the day, and as needed thro Only individuals traveling to and from the same day program should be transported together. Individuals transported together will be encouraged to be in the same groups for day services. Capacity in vehicles will be reduced to 50% capacity and efforts should be made to reduce close contact. (Individuals and staff who reside/work together in the same home may be transported in the same vehicle without a vehicle capacity restriction.) To the extent that they can medically tolerate one, individuals, staff, and the driver must wear face coverings at all times in the vehicle. For those individuals that cannot tolerate wearing a mask, social distancing must be maintained. These individuals should be transported alone or with members of the same household, when possible. Staff who cannot medically tolerate the use of a face covering should not be assigned to transport individuals. Before and after each trip, the interior of the vehicle will be thoroughly cleaned before additional individuals are transported. When appropriate, the windows should be rolled down to permit air flow and/or for vehicles using air conditioning that air recirculation be turned off so fresh air is brought into the vehicle.
Furniture has already been rearranged to provide at least 6 feet of distance between individuals. Floor markings indicate where these seats are to be located. Floor markers have been added to show the direction that foot traffic should be going up and down the hallways. All rooms will have no more than 15 individuals at a time, excluding staff. Northeast Career Planning continues to provide supported employment services. When a face to face meeting is required, both the job coach and individual will wear a face cover and maintain social distance. Prior to the meeting, the indivudal will be pre-screened with COVID questions as close to the meeting as possible. If any of the responses to the questions are yes, the face to face meeting will be canceled. The building location in Menands and Glens Falls is at 50 percent capacity and staff are maintaining social distancing, wearing face masks and staggering their time spent in their office. Intake meeings are being conducted on site and employees and indiviudals are required to wear a face mask and maintain social distance. Virutal intake meetings are available.

Transportation is provided for some of the trips. For example, a group is going up to the Catskills for a railroad trip.
The main site is located within a park. The program currently has groups meeting at parks and at various locations in the community for outdoor activities. Summer camp just ended. Starting in September, on Thursdays they will be at Leonard Park in Mount Kisco. Activities will be outdoors until it is too cold. There is also a gym located there. Another group will be going to Mount Pleasant Community Center on Thursdays. A group attends New Castle Community Center on Saturdays. the program is working on reserving space at the park where the program operates in Hawthorne for a cooking class. The program doesn't know if the bowling alley will open up again, so that is TBD. There were some other ideas such as doing a fitness Boot Camp program.

For those members that are high risk (age and/or compromised immune systems) staff are avoiding going to populated places to provide these services. All individuals will be given a fabric mask; staff have already been trained on portocols and safety. When going to individuals house, COVID-19 questions will be asked and temps. will be taken before the member can enter the vehicle. Families/households will be required to sign-off on forms that question if they have left the county within the past 30 days. Staff have been giving safety plan re-opening guidelines to families so they are kept informed. The guidance from OPWDD has very restrictive limitations on transportation. The current guidance stipulates that we may only have three passengers in a van at once. This raises both staffing and cost concerns for Triform.
Triform’s Day Hab program will be significantly different than before in its programming, staffing, length and time of day, and facilities. It is important for you to know that due to these new requirements we anticipate to only be able to provide programming from 7:45am-12:00pm each day. Due to the circumstances outlined above and in order to give Triform time to put in place all that is now necessary the earliest we will be able to reopen Triform’s Day Hab program is mid-September. In the meantime we plan to continue offering the Zoom Day Hab, aside from during the summer break from August 10-21.

Community Pre-Voc program has been operating virtually since the closure of our hub-site in March. We've been pleasantly surprised by how engaged people in that program have been and many of our business partners have been willing to participate in activities. We plan to continue operating that way until the community volunteer sites we use re-open which may be a while. Wildwood's Employment Services has continued throughout the pandemic. Many of the people we support are essential workers who have needed job coaches as they've provided needed services. Others have lost their jobs and needed help filing for unemployment and navigating the search for new work. CDC guidelines are implemented to the fullest.
