
Vaccination Registration for People with Intellectual and Developmental Disabilities
On February 5, 2021, Governor Andrew M. Cuomo announced that all New Yorkers 16 or over with Intellectual or Developmental Disabilities living in non-certified settings will be added to the 1b prioritization category and will be eligible to receive COVID-19 vaccines as of February 15, 2021.

Registration Information
To receive a COVID-19 Vaccination appointment, all persons with IDD 16-65 years of age must register at the New York State-Operated Vaccination Site using the Am I Eligible app. To check other eligibility, click here.
Follow these steps to schedule and receive your COVID-19 Vaccination
*LIFEPlan Care Managers are available to assist members with scheduling and preparations to receive their COVID-19 vaccination.
Step 1: Determine eligibility and schedule an appointment
- Check if you are eligible and schedule appointment here.
- Once completed, you will receive a confirmation email with a barcode.
- Bring confirmation email to your scheduled appointment. If you don’t have email, contact your County Health Office for assistance.
Step 2: Complete a Vaccine Consent Form
- Complete the New York State Covid-19 Vaccine Form to receive a submission ID.
- Bring the submission ID to your appointment. If you are unable to access online, the Vaccine Consent form is available at vaccination sites.
Step 3: When you arrive for your Vaccine appointment, you must present proof of eligibility.
Proof can include:
- Any legal proof of date of birth and residency
- Letter to confirm IDD status can be obtained from LIFEPlan CCO Care Manager (If a person has no other acceptable proof, such as a notice of decision from OPWDD, Care Managers can provide a letter stating that a person is a member of LIFEPlan through email or other means).
- Employee ID card
- Letter from an employer or affiliated organization
- Pay stub
- Driver’s license
- Passport
When you arrive for your appointment, you will be asked a series of clinical questions to ensure readiness for a vaccine. You will be asked for insurance information but the vaccine is free.
Step 4: Second Vaccine
The second dose appointment is automatically scheduled when you receive the first vaccine dose, for the same time and same location 21-28 days after the first dose depending on which vaccine is received.
You will receive a card at the time of your first vaccine with the date and time for the second vaccine appointment. A confirmation email will follow a few days later.
