Regional Social Care Networks Announced
On August 7, 2024, Governor Hochul announced the nine organizations selected to create Social Care Networks (SCNs) under the New York Health Equity Reform (NYHER) 1115 Waiver Amendment. SCNs will be responsible for coordinating the delivery of Health-Related Social Need (HRSN) services to Medicaid members. SCNs will develop and maintain a comprehensive network of community-based organizations (CBOs) to support the delivery of HRSN screening, navigation and “Enhanced” HRSN services to eligible members. Such services may include housing, transportation, nutrition, and care management. Non-profit developmental disability providers are considered a CBO under this program.
All Medicaid Members are eligible to receive HRSN Screening and Navigation services. People with Intellectual and Developmental Disabilities (IDD) who are enrolled in NYS Mainstream Medicaid Managed Care are among the populations eligible to receive the “Enhanced” HRSN services. Under this 1115 program, the IDD criteria are NOT dependent upon OPWDD eligibility. Approximately 230,000 Medicaid Managed Care members with an IDD have been identified statewide.
The SCNs have access to $500 million in total funding to support SCN start-up and infrastructure costs, including providing resources for capacity building to CBOs. Over $3 billion has been allocated to fund HRSN-enhanced services.
