Whistleblower Responsibilities and Protections

A whistleblower is a person who voluntarily provides information about dishonest or illegal activities occurring at an organization. Organizations include a government department, a public company or a private organization that bills Medicaid for services.

Whistleblower protections must be in put place, per the Whistleblower provisions of the NYS and Federal False Claims Acts. All LIFEPlan CCO employees, board members, volunteers, and contractors are protected from any form of retaliation, if they make a report in good faith. LIFEPlan CCO will promptly act to investigate and correct any reports of improper activity.

During the period of investigation, the identity of the Whistleblower, if known, shall remain confidential to those persons directly conducting the investigation, unless the issue requires involvement of law enforcement.

LIFEPlan CCO is committed to preventing, detecting, and correcting any fraud, waste, abuse or improper conduct in Medicare, Medicaid, and other health care programs.

If you believe fraud, waste, abuse, or other improper conduct has occurred, you are required to report right away. You can make a report in any of the following ways:

· Call and leave a voicemail on the Compliance Hotline at 315-338-4095;

· Contact the Corporate Compliance Officer using your LIFEPlan CCO email account only: laura.wells@lifeplanccony.com;

· Send a confidential or anonymous letter to the Corporate Compliance Officer: Laura Wells 939 NY-146, Clifton Park, NY 12065;

· Contact your Supervisor or any Supervisor or Director; or

· Contact any member of the Compliance & Quality Assurance Committee.
