Member Support Liaison Drop-In Sessions: Find Support With Someone Who Has Been There
As part of our ongoing support, we would like to introduce you to an important resource in your Care Management journey – the Member Relations Liaison.
A Member Relations Liaison is a family member of or an individual who has received Care Management services from LIFEPlan and has navigated a similar journey to yours. Having the option to connect with someone who shares life experiences with you can be incredibly helpful. Whether you’re looking for guidance, advice or simply someone to listen to, our Liaisons are here to help.
Here are some areas of expertise that our Liaisons may be able to support you with:
- Future Planning: Helping you plan for long-term care and services
- Insurance or Service Denials: Assisting with issues related to insurance or denied services
- Durable Medical Equipment: Guidance on obtaining and using necessary medical equipment
- Behavioral Support: Advice and resources for managing behavioral challenges
- Educational Advocacy: Support in navigating the education system and advocating for appropriate services.
As a parent, caregiver or member, the Member Relations Liaison understands your journey and is ready to offer the support you need. We encourage you to reach out to Member Relations with any questions, concerns, or if you would like to connect with a liaison.
Liaison Drop-in Session Dates for 2026
We invite you to join us for our one-hour Liaison Drop-In Sessions where you can connect with our member services liaisons to share your thoughts, ask questions, learn about resources, and connect with your peers.
Every quarter on Thursdays, 12 p.m. – 1 p.m.

Stacy Mason | 845-234-4221
I’m the mom of two sons with autism in the Hudson Valley. I am a Licensed Clinical Social Worker (LCSW) and Parent Trainer for families who have children with IDD. I am knowledgeable about the OPWDD system, self-direction, early intervention, the education system, the autism spectrum, special diets, and resources for individuals with IDD in the Hudson Valley. I love to help other families navigate their journey with their special needs loved one and welcome your phone calls with any questions, concerns, or feedback.

I’ve navigated the services system with my son Patrick for 25 years. I have a passion for helping other families of children with IDD in getting the support services they need.
As a Liaison, I enjoy hearing from self-advocates about their wants, needs, and desires when it comes to disability services. I look forward to sharing information that can help them grow and become a part of their communities.

I have a 21-year-old daughter Veronica who has Down syndrome. I enjoy encouraging other families new to IDD services and sharing available resources that may help them.
As a Liaison, I have the chance to connect to families who are unfamiliar with disability services. It is so fulfilling to point them in the right direction and help them find what they need.

My middle son Nathanael receives IDD services. In addition to working part-time as a Liaison, I’m a self-directed start up, Support Broker, and a New York State Housing Navigator.
I started to volunteer in the inclusive special education classroom in high school. When I became a mom to an extra needs child, I knew what I was being prepared to do in life. I could now understand what other parents felt and needed. As a Liaison, I am able to expand my knowledge and help other individuals and families dream, plan and achieve.
