New Member Orientation

New members are invited to attend a bi-monthly Orientation meeting, held virtually, to learn basic information about Care Management and LIFEPlan, and to have the opportunity to ask questions.

Member Support

Members are supported throughout their journey by numerous Member Relations programs. Working alongside care management teams, Member Relations offers a Member and Family Advisory Council, connection to resources, and peer support to address barriers or concerns experienced by members and their families.

Member and Family Advisory Council

The Member & Advisory Council (MFAC) comprises LIFEPlan CCO members and families serving as the voice of members. It is a diverse group that reflects the diversity of the membership. MFAC offers a forum to share ideas on topics important to our members, provide input, and share their experiences and unique perspective to enhance policy and program development that affect the care and services individuals receive.

The MFAC provides a mechanism to:

  • Seek and learn from the member and family perspective
  • Promote a culture of person-centered care
  • Guide policy and program development through collaboration to improve practices
  • Enhance the delivery of high-quality services

Regional Liaisons

The Member Relations Liaison is a point of contact for LIFEPlan members and families when you need another person to listen and guide you to answers. As team member of the Member Relations department, the liaison is also a LIFEPlan member or parent caring for a child with a disability. Members are encouraged to reach out to Member Relations with questions, concerns, or support. As a parent or caregiver, the Liaison understands the journey and is ready to help.
