LIFEPlan CCO’s Community Networking Events bring together Care Managers, providers, community organizations, and families to share resources, build relationships, and strengthen coordinated, person-centered care.
We are honored to be adding a wonderful new feature to events this year. We will be featuring, at select locations, Unseen, a powerful documentary that highlights the lived experiences of individuals with IDD and their families, grounding each gathering in real stories, reflection, and shared understanding.
At these events, you can:
