2024 LIFEPlan Art Gallery Submission Form

Each year, LIFEPlan virtually showcases the artwork of many of our talented members on our website Art Gallery. We are now accepting submissions to be featured on this year’s Member Art Gallery. Share your artwork, including paintings, drawings, sculptures, or any other creative masterpieces.


Art will be accepted from November 11 through November 25. Submit one photo of your art to be featured. In order to be featured, you must submit a signed consent form with your artwork. If you need assistance, please reach out to your Care Manager.
