What is Care Management?

Effective July 1, 2018, The Office for People with Developmental Disabilities (OPWDD) is making changes to how services are coordinated for individuals with Intellectual and Developmental (I/DD) disabilities. Medicaid Service Coordination is now being called Health Home Care Management.

Health Home Care Management is a new way to coordinate care, combining developmental disability services and supports with health and wellness services.

Health Home Care Management will be provided by a Care Coordination Organization (CCO).

Our Goal

Health Home Care Management will integrate and coordinate long-term supports and primary, acute and behavioral health care to address the needs of the whole person.

Through Care Management, the IDD population should see:

  • Improved outcomes through care coordination including healthcare, personal and social supports
  • Improved disease-related care for chronic conditions
  • Improved access to preventative care

LIFEPlan CCO NY, LLC has been initially designated as a Care Coordination Organization/Health Home (CCO/HH) to serve Individuals with Intellectual and Developmental Disabilities (I/DD). LIFEPlan CCO's vision is to be the CCO of choice, upholding our core values- Person-Centered Services and Supports, Community Inclusion and Participation, and Independence and Self Direction.
