We are proud to launch the Care Management Impact Series, a new video and blog initiative that highlights the incredible work happening every day within our organizations.
Over the past few months, our team has met with care managers, members, and families across New York State to hear their stories firsthand. These conversations have reminded us of our “why”- the reason we do what we do, and how meaningful care coordination truly transforms lives.
Each story in this series showcases how care managers work side by side with members and families to create opportunities for independence, inclusion, and connection. Just as importantly, these stories reflect how our members and families inspire us in return, shaping our understanding of what it means to live a truly person-centered life.
Every month, we’ll be sharing new written and video stories on our website and social media channels, featuring voices from across our network. We invite you to follow us, subscribe to our YouTube channel, and hear directly from the people whose lives are changed through care management.
Meet Jeremy & Kim
Our first feature spotlights Cheryl, a devoted mother and caregiver; Jeremy, her son, who receives supports through LIFEPlan CCO; and Kim, their care manager, whose dedication helped connect them to services that truly changed their lives.
Through Kim’s guidance, Jeremy now participates in community classes through self-direction, receives community habilitation, and has home modifications underway to help him live safely and independently. Cheryl also found new stability and peace of mind through the CDPAP program- allowing her to care for Jeremy while supporting her family.
As Cheryl shares, “If Kim could be cloned, I’d do it. She’s changed our lives.”
