That’s why people with intellectual and developmental disabilities (IDD) who enroll in LIFEPlan Care Management get help finding, coordinating, and accessing the services they need.

Through close, personal support, Care Managers coordinate healthcare, personal services, social supports, disease-related care for chronic conditions, and access to preventative care. Care Management services are provided with sensitivity to cultural and language preferences.

O Gestor de Cuidados

The Care Manager helps Members in many aspects of life, including planning for IDD services, maintaining services such as Medicaid, SSI, and other social services, assisting with healthcare and medical appointments, self-direction, and any services identified in your Life Plan. Care Managers stay in contact with a regular schedule of meetings each quarter (depending on the level of service chosen). Members who contact their Care Manager can expect to hear back from them within 48 hours, except in an emergency.

O Plano de Vida

The Life Plan is a tool that guides choices and coordinates services across systems. The Care Manager and the person with IDD create the Life Plan with natural supports such as parents, siblings, and other caregivers. The Life Plan takes a personalized, comprehensive look at needs, wants, preferences, and strengths. When complete, the Life Plan guides the delivery of developmental disability, medical, and behavioral health services. It is regularly reviewed by you and your Care Manager.

Serviços OPWDD

OPWDD services support a range of basic and enhanced needs, including residential, employment, community habilitation, day habilitation, and respite services. Persons with intellectual disabilities, cerebral palsy, Down syndrome, autism spectrum disorders, Prader-Willi syndrome, and other neurological impairments can apply to receive OPWDD-funded services. These services are delivered directly and through a network of approximately 500 nonprofit service-providing agencies
