Help for Families with Disabilities

Are you a person, or a family member of a person, who has been diagnosed with a developmental disability? A large network of nonprofit service agencies, including Care Coordination Organizations (CCO) offer programs, support, and services that include employment, community and day programs, clinical services, residential programs, and family respite services, among others.

We are Ready to Help!

LIFEPlan CCO assists families with obtaining these services through our Enrollment Specialists, who understand the process and assist with enrollment and eligibility. There are three main requirements.
