Jean

Jean Jacobson (“Dr. Jean”) | Clinical Director | Mohawk Valley

Educational/Work Background: I attended University of Scranton and got a BS in Math with a minor in Business. I worked as a data entry clerk, administrative assistant, IT manager, and was promoted to work on a system analyst team for a major investment banking firm in NYC. At age 27, I changed careers and took night classes to apply for a Ph.D. program in Clinical Psychology at University of South Carolina. I did my post-doctoral internship at SUNY Stony Brook in their Autism Center. Since then, I have worked as a psychologist, consultant, program director, and managing director in various programs for people of all ages with IDD. I joined LIFEPlan CCO in 2019 as an opportunity to advocate for needed changes in the service delivery system. I grew up on Long Island with an older brother with autism and chronic mental illness, and my nephew has intellectual disability, autism, and cerebral palsy. They are my everyday inspiration!

Fun Fact:  I love taking care of my 5 rose bushes that I planted last year (plus a bunch of other perennials and flowering shrubs).

Carrie Loubier | Assistant Clinical Director | Mohawk Valley

Educational/Work Background: I have been a registered nurse since 2001 (St. Elizabeth’s College Of Nursing). Several years ago I became a Certified Case Manager. I have worked in multiple fields since obtaining my nursing degree including Madison Cortland ARC, Upstate Cerebral Palsy, home care, and Case Manager and Utilization Management Nurse for Excellus  (compared requests for services against policies to determine if the request was medically).

Fun Fact: I am a wife and mother of 3. My hobbies include cooking, baking, crafting, camping, and bowling.

Anna Fields | Clinical Support Coordinator | Central New York

Educational/Work Background: I graduated in 2019 from SUNY Albany with my Masers in Social Work and went on to get my license. I have worked in human services and with the IDD population since 2013 as a Direct Support Professional, Fiscal Intermediary Coordinator, Care Manager, and now Clinical Support Coordinator with LIFEPlan CCO.

Fun Fact: I originally went to college to study art but ultimately did not pursue that career path. However, I am still a multimedia artist and primarily design tattoos and ink drawings.

Kesha Penree | Crisis Specialist | Mohawk Valley

Educational/Work Background: I received my BS in Psychology – Child Life from Utica College. Following college, I was employed at the Kelberman Center developing and running recreation programs for individuals with Autism. I began my career at LIFEPlan CCO as a Care Manager in 2018 and eventually transitioned into a Supervisor role. Currently, I am the Crisis Specialist on the Clinical Team.

Fun Fact: I am raising two wonderful children, Lucas and Georgia, with my husband. We also just added a puppy to our family, Gaby, a lab/shepherd mix. Even with all the chaos being a mom brings, I also manage to keep over 60 houseplants alive, a pandemic hobby turned source of relaxation.

Patti Geary | Health Promotion Specialist | Mohawk Valley

Educational/Work Background: Before my nursing career, I was a Certified Medical Assistant, working in medical offices and at St. Elizabeth Hospital medical residency program. I graduated from MVCC-Utica and attended Utica College. I wore many hats as a nurse, including medical offices, intensive rehab unit, disability consultant, ambulatory services at local hospital and most recently Excellus BCBS in Utlization Review and Case management, becoming a Certified Case Manager. I joined LIFEPlan in April 2021, to work with our growing clinical team.

Fun Fact: I have taken piano lessons for the past 5 years and have 2 recitals each year, with the other adult students. I also love to do diamond painting, and I am an avid reader.

Julie Baum | Clinical Specialist & Care Manager | Mohawk Valley

Educational/Work Background:  I have a Mass Communications degree with a minor in Women’s Studies from SUNY Plattsburgh, but even at that time, I knew that I wanted to work with people in Human Services. I have worked with people with IDD for over 30 years in Residential Services, Medicaid Service Coordination, Respite, Community Habilitation, Supported Employment and FSS programs at The Arc Oneida Lewis and Upstate Cerebral Palsy.

Fun Face:  I love the outdoors! I have completed the 90-mile Canoe Race from Old Forge to Saranac Lake 8 times. I love to camp, garden, fish and paddle.

Alison Delnero | Clinical Specialist |Mohawk Valley

Educational/Work Background: I have a bachelor’s degree in psychology from Hood College in Maryland. Prior to coming to LIFEPlan I worked at upstate cerebral palsy as a behavioral specialist assistant, a case manager for child protective services and most recently a hospital discharge planner.

Fun Fact: I was born and raised in Baltimore, Maryland. Now, I live with my husband, son, 2 dogs and chinchilla in Rome, NY.

Chris Washburn | Clinical Specialist | Southern Tier

Educational/Work Background: I am an LMSW. I received my MSW degree from Marywood in 1991. I worked at HCA since 1992 as a Social Worker and MSC Supervisor. We transitioned to LIFEPlan CCO Care Coordination in 2019. I began my career at LIFEPlan CCO as a Care Manager Supervisor. Currently, I am a Clinical Specialist and I continue to provide care management to a few members.

Fun Fact:  I live with my husband and “our flock”! We have a green wing macaw, cockatoo and cockatiel. A fun fact about myself is that I enjoy dancing and I belong to a dance studio where I dance tap and jazz with other adult women.

Tynirra Mollette | Clinical Specialist | Hudson Valley

Educational/Work Background: I have a bachelor’s degree in psychology (with a minor in sociology) from Stony Brook University.  I decided to also pursue a master’s degree in social work at Stony Brook University as well.  I’ve been in the social service field since 2007.  I am a compassionate social worker aiming to enhance overall well-being and help meet basic and complex needs of communities and people. I am skilled in advocacy, organization, communicating, active listening, and critical thinking and committed to providing emotional and social support to promote overall wellness.

Fun Fact:  If I did not become a social worker, I would of became a veterinarian.  I am also a member of the BEST sorority ever, Sigma Gamma Rho Sorority Incorporated!
