Care Management Services

At LIFEPlan CCO, we believe that every person should have the right to define the life you want to live.

That’s why people with intellectual and developmental disabilities (IDD) who enroll in LIFEPlan Care Management get help finding, coordinating, and accessing the services they need.

Through close, personal support, Care Managers coordinate healthcare, personal services, social supports, disease-related care for chronic conditions, and access to preventative care. Care Management services are provided with sensitivity to cultural and language preferences.

New York State Mental Hygiene Law, Article 33 (Rights of Patients)

Article 33 ensures that people receiving mental health or related services are treated with respect, have access to quality care, and retain fundamental rights. It also sets clear rules for safety, confidentiality, and accountability.

Independent ombudsmen exist to help patients resolve complaints about mental health or developmental disability services.
If you have concerns about your services, need support trying to get services or need help understanding your rights, you can contact the Independent Ombuds Program (IDDO).

This advocacy program is independent from OPWDD and run by the Community Service Society of New York (CSSNY).

The helpline is free and confidential: 1-800-762-9290 (Monday-Friday, 9:00AM-5:00PM). Learn more at iddony.org

For more information on your rights in New York State, visit the Department of State’s website.

The Care Manager

The Care Manager helps Members in many aspects of life, including planning for IDD services, maintaining services such as Medicaid, SSI, and other social services, assisting with healthcare and medical appointments, self-direction, and any services identified in your Life Plan. Care Managers stay in contact with a regular schedule of meetings each quarter (depending on the level of service chosen). Members who contact their Care Manager can expect to hear back from them within 48 hours, except in an emergency.

The Life Plan

The Life Plan is a tool that guides choices and coordinates services across systems. The Care Manager and the person with IDD create the Life Plan with natural supports such as parents, siblings, and other caregivers. The Life Plan takes a personalized, comprehensive look at needs, wants, preferences, and strengths. When complete, the Life Plan guides the delivery of developmental disability, medical, and behavioral health services. It is regularly reviewed by you and your Care Manager.

OPWDD Services

OPWDD services support a range of basic and enhanced needs, including residential, employment, community habilitation, day habilitation, and respite services. Persons with intellectual disabilities, cerebral palsy, Down syndrome, autism spectrum disorders, Prader-Willi syndrome, and other neurological impairments can apply to receive OPWDD-funded services. These services are delivered directly and through a network of approximately 500 nonprofit service-providing agencies
