The Link to Better Health program is funded by a grant from the CDC in collaboration with the New York State Department of Health. Eligible individuals receive 90 days of free support from a Linkage Coordinator who can help connect with healthcare services.
To be eligible, a person must:
- Be diagnosed with an intellectual and/or developmental disability (IDD)
- Live in New York State
- Be over 18 years old
- Have unmet healthcare needs
- Not be currently receiving care management services through a CCO (individuals can be working with a CCO on the OPWDD eligibility process, they just can’t be enrolled and receiving Care Management services from a CCO)
Benefits for this free 90-day program include:
- Help finding medical providers and coordinating medical appointments
- Connect with social services, such as SNAP, HEAP, and more
- Help navigating the healthcare system
- Connect to preventative services, such as screenings and well-care visits
