Care Managers and Providers Working Together

Judy Jarosh, Intake Coordinator at the Arc Chemung Schuyler, shared the story of a member who successfully moved into an IRA in Tioga County with the support of his LIFEPlan Care Manager, Kari Martin.

Despite Kari’s active support over several years – helping the member to gain medical care and food, and to maintain regularly scheduled psychiatry appointments – the member found himself at risk of becoming homeless, struggling with food insecurity, and often experiencing social isolation.

Kari and Ms. Jarosh worked together closely to assist the member through the Certified Residential Opportunity process, including taking him to a potential IRA setting for several visits and ensuring that his voice and interests were heard throughout.

A year later, the member is flourishing in his new home at Arc Chemung Schuyler as he is supported by the DSPs, residential team, and Kari. He has formed friendships and found social connections through his artwork, which reflects his improved mental health since moving into his new home.
