The Member Relations Department is your support team, and we’ll be here with you every step of the way.

Engage With New Member Orientation

All new members are invited to attend orientation meetings, which are held virtually. We’ll walk you through all the foundational information to help get you started.

This is your introduction to Care Management, disability services, and LIFEPlan programs with clear, and easy-to-understand information that will help give you the confidence you need to take the next steps. This is also an open forum to ask questions with your peers to help plan for your unique needs and goals.

Join Our Virtual Learning Sessions

Our Virtual Learning Sessions (formerly known as Member and Family Forums) are held monthly on topics of interest to members, families, and others in the IDD community.

Popular topics for our Virtual Learning Sessions include enrollment, eligibility, and accessing disability services in New York, understanding Medicaid, Medicare, and HCBS supports, guardianship, managing life transitions, and future planning. LIFEPlan Members are notified of upcoming sessions via our Member E-news.

Meet Your Peer Supports: Our Member Relations Liaisons

Our Member Relations Liaisons are parents or caregivers of members who understand the journey and can share in your experience.

Sometimes your best resource is someone who has been there. That is why we host quarterly, one-hour online “Drop-In Sessions” with liaisons from across the state. They can help you understand your options, prepare for meetings and decisions with plenty of opportunities to share your thoughts, ask questions, and find resources.

Participate in Our Self-Advocate Community

Are you a self-advocate and want to meet other LIFEPlan members like you and make some new friends?

Self-Advocates receiving services from LIFEPlan are invited to attend monthly meetings to be a part of the LIFEPlan’s Self-Advocates group. Join us to share resources, discuss concerns, compare notes, and support each other virtually. We encourage you to bring your thoughts and ideas to an introductory meeting.

Join the Member and Family Advisory Council

The Member and Family Advisory Council (MFAC) is comprised of LIFEPlan members and families serving as the membership’s voice. MFAC offers a forum to share ideas on topics important to our members, provide input, and offer their experiences and unique perspectives to enhance policies and program development that affect the care and services that members receive.
